Efforts aimed at preventing hospitalizations due to heart failure (HF) should be patient-
and community-centered and have a focus on outpatient management of HF.

Outpatient intravenous (IV) diuretics have been shown to be safe and effective in
treating volume overload in patients with HF, but most patients with HF lack access
to same- or next-day outpatient assessment and IV diuretics.

Advanced-practice providers (APPs) play an important role in providing team-based
care for many cardiovascular diseases and can be a valuable part of improving access
to and coordination of HF care.

We describe our experience in establishing a novel APP-led, urgent-access HF clinic
to provide timely assessment and outpatient IV diuretics for patients with signs and
symptoms of decompensated HF.Key Words
