Advanced Practice Provider Urgent Outpatient Clinic for Patients With Decompensated Heart Failure

      Efforts aimed at preventing hospitalizations due to heart failure (HF) should be patient- and community-centered and have a focus on outpatient management of HF.

      We describe our experience in establishing a novel APP-led, urgent-access HF clinic to provide timely assessment and outpatient IV diuretics for patients with signs and symptoms of decompensated HF.
