Power Spectral Analysis of Heart Rate Variability During the Postural Change Reveal Exercise Capacity From Cardiopulmonary Exercise Testing

      Background: Orthostatic hypotension related to impaired sympathetic activity was reported as a prognostic factor of chronic heart failure (CHF). Power spectrum analysis (PSA) of heart rate variability (HRV) is a noninvasive technique providing a dynamic assessment of sympathetic activity. Few data is available which associates PSA during postural change with CHF. Methods: We evaluated sympathetic activity with PSA of HRV of 206 CHF patients calculated by low-frequency (LF), high-frequency (HF) component during postural change. We divided them into 4 groups by median of LF/HF at sitting (1.89) and upright (3.93), and compared background and exercise parameters. Results: There is significantly lower BNP level and higher exercise tolerance in patients with higher LF/HF at upright position. Conclusion: Reduced sympathetic activity documented by postural change perceptively predicts impaired exercise tolerance in CHF.
