Spot Measurement of Urine Sodium as an Index of Natriuretic and Diuretic Response to Intravenous Furosemide Infusion in Acute Decompensated Heart Failure

In logistic regression analysis, UNa <55 mM (OR: 17.1, 95% CI: 2.5–351.6, p = 0.002) and FENa <2.5 (OR: 8.3, 95% CI: 1.8–46.9, p = 0.006) predicted net negative fluid balance <2,250 mL over subsequent 24 hours. Conclusions: In patients with ADHF treated with continuous infusion of furosemide, natriuretic response as determined by spot check UNa or FENa may predict subsequent amount of diuresis.
